Hello, 

During the month of February 2013, 5th graders will be receiving their 6th grade physical and dental forms for the 2013 school year; 1st graders will be receiving their 2nd grade dental forms and 4 year old pre-school students will be receiving their kindergarten physical, dental and eye exam forms. These exams are due on Packet Day in August or anytime if completed sooner. 

I am still missing a few dental exams from this year. Reminders were sent home in December and in March. Per the state mandate, the school may hold the child's final report card if the dental forms for the 2012-2013 school year are not received by May 15th. Please schedule your appointment now to avoid the above. If you require a new dental form, please contact me.

Please remember, all medications, either perscription or over the counter need a doctor authorization form. All medications must be in the orginal container. Forms for medications may be given out upon request.

On picnic day, I will be returning all students' medications. Please check their backpacks that evening. If you would like to pick up their medication yourself, please see me that day.
